We currently focus in the area of Community Living Waiver for Adults ages 18 and older with primary diagnosis of Intellectual disability and/or dual diagnosis, that can also include mental health (funded through the Community Living Waiver).

IMPACT relies on several components during the referral process, which include but are not limited to:

• Verbal and written communication with ID Support Coordinator to include the referral form, current psychiatric evaluation, comprehensive assessment, and a completed admissions application.
• If the individual meets qualifications for possible placement, a tour will be scheduled for approved locations and/or the provider and will schedule an onsite visit to the individual’s current location.
